The clinician requested an Upper Removable Appliance (URA), with Adams clasps on the 6D / D6 in 0.7mm S.S wire and an upper central prosthetic tooth.

Palatal andLabial view of the upper working model.

Firstly, a suitable upper central prosthetic tooth is waxed into place as shown in the picture.

Note: - a small wax flange / gingival margin is created with the wax.

A plaster matrix is created around the prosthetic tooth and the adjacent teeth by mixing up a small amount of plaster and forming it around the outlined area.

Note: - The plaster matrix should cover the incisal tip of the prosthetic tooth, to prevent any movement / dislodging of the tooth and to assist accurate relocation after boiling out.

Once the plaster matrix has set, the wax gingival margin is boiled away using a boiling out machine, the Adams clasps waxed in to place, the prosthetic tooth is relocated in the plaster matrix (fix with a 'spot' of super glue if necessary) and the model is then soaked for 10minutes prior to spraying of the acrylic.

After soaking, the base plate can be sprayed up with cold cure acrylic in the usual way. Remember to spray pink denture acrylic around the tooth / gingival margin area to help the prosthetic tooth blend in to the natural dentition. The remaining base plate is sprayed in clear cold cure acrylic and then placed in a hydroflask for 10 minutes until it is cured.

Once cured, the model / appliance is 'boiled off', removing the wax from the appliance and upper working model. The appliance is then ready to be trimmed and polished.

These pictures show the final appliance trimmed and polished and in place on its model prior to dispatch to the clinic.
